GIRARD - Michael K.J. Oslavic, 15, died at 4:05 p.m. Monday, April 16, 2012, at the Cleveland Clinic Children's Hospital, following complications from heart and lung disease.
He was born July 25, 1996, in Warren, the son of Michael J. Oslavic and Joette L. Sondheimer.
He attended Girard Junior High School and was a member of Churchill United Methodist Church.

Survivors include his father of Fort Hood, Texas; mother, Joette L. Sondheimer of Girard; and a brother, Brian K. Oslavic, at home.
He was preceded in death by his grandparents.
Funeral services will be held at 1 p.m. Saturday at Churchill United Methodist Church, where the family will receive friends one hour prior to the services.
Arrangements are being handled by the Staton-Borowski Funeral Home, Warren.
